Avid Quality Care is an HCS Provider. We have group homes and foster care / host homes for individuals with intellectual and/or developmental disabilities. These range from Autism to Down Syndrome to Traumatic Brain Injury and everything in between. We have consumers that drive and work full time jobs all the way to consumers that need 100% full care including feeding, and bathing.
We are responsible for making sure we hire and train staff to take excellent care of these individuals. We have to be sure all of their medical needs are being met in a timely manner. We also have to implement any other service that might benefit the individual and help them to be more independent.
This nurse/ case manager not only responsible for the face-to-face exams but this nurse must be able to keep up with all the paperwork in an organized fashion and be able to communicate effectively with the rest of the team members, it also requires comunication with parents, guardians and Local authority. You have to keep comunication with therapist and with all of the staff/providers that deliver services to our clients. Lots of documentation is required with this position. Your direct supervisor is the Director of Nursing who is also our HIPAA compliant officer. You also need to be able to have a positive relationship with the other LVN/case manager's and RN so that you can work as a team for the good of the clients and their families. You will be responsible for communicating with the clients doctors and keeping thorough, current and accurate records. .
This position is Monday - Friday 8am-5pm but sometimes is required that you stay some extra hours.
We are a drug free work place and you much pass a drug test before you are hired. You also have to pass the back ground check and have to verify that your nursing license is in good standing. We also check the nurses aid registry. You must have a valid drivers license and current auto insurance. You will need to be up to date on CPR and First aid as well. You will also be required to complete PMAB training to deal with possible physically aggressive behaviors.
You are responsible for visiting the clients home, work, school, dayhab or any other setting the client may visit regularly. You will need to use your own personal vehicle but Avid does reimburse mileage.
Must have computer skills and have experience on Microsoft Office.
